SEPA direct debit mandate

I hereby authorize CHIMFUNSHI e.V., creditor identification number: DE45ZZZ00001159473 to collect the above payment from my account by direct debit. At the same time, I instruct my account provider to pay the direct debit amount from my account to CHIMFUNSHI e.V. The mandate reference number is to be communicated separately.
